Raise a report of the incident.
Explain

By raising the report of the incident it may lead to an investigation to find out what was the cause of this and not who has done it. This will be entered onto the Error Management System and then allow for the trained investigators to see what had caused the incident.

This resulted in minor damage to the aircraft rear fuselage and required a seal to be replaced on the aircraft.
Explain

There was damage found on the aircraft which would have to be documented and the repairs carried out to get the aircraft back to its serviceable status.

Staging removed from service and quarantined due to concerns over its failure to maintain the selected height.
Explain

we would have to remove the possible faulty staging from use due to the fact it had caused damage to the aircraft. We would also need to find out if the staging is in use any where else on the base, company and possibly world wide. This would be best approached by contacting the manufacturer of the staging and highlighting to them that there had been an incident with this type of staging supplied for use on the aircraft.

Proposed and looked at the introduction of a secondary fail safe features for the staging and then assess if fit for purpose.
