Slide 29 – TWO TYPES OF ERRORS
failures and local triggering events
This is where incidents, accidents and or occurrences happen straight away, the image shows a vehicle losing its roof and back door as it was driven under the Tornado wing.
Here we have a Tornado that went to select undercarriage down using hydraulics. This fails due to a fault, so the pilot selects the emergency system of the blow down using a nitrogen bottle to operate the undercarriage. This had been fitted or left with no gas in it and this resulted in the latent failure and the aircraft landing on its fuselage.
